What Is Fascia & How Is It The Driver Of Your Chronic Pain?

 

Fascia is the connective tissue that joins every part and system within your body. It can directly send pain signals because it has pain receptors capable of translating a mechanical stimulus into painful information, and if there are dysfunctional mechanical stimuli (dysfunctional movement), receptors which receive stimuli from within the body can become pain receptors. In other words, if you’re moving in a dysfunctional manner, your fascia will grow MORE pain receptors. The pain receptors then create substances that can alter the surrounding tissue and cause inflammation. Parts of the fascia come in contact with these inflammatory molecules and can cause sensations of pain, creating a vicious circle of inflammation and chronic pain. There is not nearly enough discussion around this feedback loop and it’s massive role in chronic pain.

All the fascial layers need hyaluronic acid to slip one on the other. Decreased quantity compromises the sliding of connective tissues. There is much evidence that the change in the viscoelasticity (having a sticky consistency between liquid and solid and also being elastic) of the fascial system is an important cause of chronic pain. Hyaluronic acid would become sticky, adhesive and less lubricating, altering the lines of forces within the different fascial layers. This mechanism could be one of the causes of joint stiffness and pain that is particularly bad in the morning.

The sensitisation of pain receptors could also be caused by the local reduced blood flow resulting from fascial tension, which prevents the proper functioning of the skeletal muscle and creates, for example, trigger points of hardened, immobile fascia (Bordoni et al., 2021).

Fascia Becomes Dysfunctional When Movement Is Dysfunctional

So What Is Dysfunctional Movement?

To put it very simply, the body has evolved to move in a certain way. This is the hierarchy of movement that functional patterns is based upon: standing, walking, running and throwing. If these movements are performed correctly, fascia responds extremely well. The problem is that not many people know how to perform these foundational movements well. Functional patterns has broken down the gate cycle into its most basic components to encode them into peoples movements through precision and repetition.

To add to this, people have recently developed many popular ways of moving, exercising and gaining muscle that are highly dysfunctional. This correlates with the increase in chronic pain, fatigue and early-onset chronic disease. A good way to tell if a movement is dysfunctional or not is the ‘gate cycle test’. If movement does not directly make you run faster, it is likely dysfunctional. For example, back squatting 100kg will make your legs bigger, but those gains do not directly correlate to the track field.

Conversely, immobility was also listed as a key driver of fascial dysfunction. The issue is, once chronic pain and fatigue set in, people are far less likely to move. The solution is to give people postural exercises to begin with. These exercises directly relate to the gate cycle but are very gentle on the body. They are highly precise and can undo the initial dysfunction that lead to the immobility in the first place. From there, people go from being in chronic pain and exhausted, to exercising multiple times a week without any pain.   • Your first session will be 90 minutes long.

    Our comprehensive initial consult involves a gait and/or activity assessment, where we analyse your gait cycle in slow motion and highlight dysfunctions present. This is a crucial part of the appointment as it enables us to tailor a program that suits your body and not just a cookie cutter approach. Following this, we will take you through myofascial release techniques that may help you to get on top of any dysfunctions we have pointed out. Specific postural correctives, chambers and dynamic exercises may follow depending on how your body handles each level of exercise.

  • What Happens After My First Session?

    So you have completed your initial assessment and session, what now? Your Biomechanics Instructor will discuss their recommendations while taking into consideration your unique situation and their assessment findings. Although estimating the duration of improvement of scoliosis can be like predicting the length of string, we do know that the more time you are able to spend doing your Functional Patterns training, the faster you will make progress. Most protocols include 2-3 60 minute sessions per week alongside homework. You can also include some group classes to practice your fundamentals.

    Following your initial consult, we will continue to build on the techniques you learned through chambers, correctives and dynamic exercise. As gait cycle results are our number one metric to judge objectively your progress, we will usually take video again of running/sprinting/your specific activity during these sessions. Throughout your sessions, we will work on breaking down the dysfunctions that lead to your scoliosis and pain while building new, functional patterns.
